Community Support Services
Community Support Services (CSS) has been operating in Annapolis County providing Independent Living Support (ILS) for persons with disabilities and long-term mental health concerns since July 1994.
Mission Statement
The mission of CSS is to provide staff support in individually identified areas of need on a regularly scheduled basis to adults with intellectual disabilities or long-term mental health challenges, in order to assist each person to live as independently as possible within the community.
Goals

To provide individualized, client centered and advocacy-oriented service planning and delivery.

To provide support which enables each person to live as independently as possible within their community.

To provide continuity of support.

To provide crisis intervention.

Through teaching and assistance, to support and enable each person’s continued growth and quality of life.

To support and enable the persons we support to make informed decisions and self-advocacy.

To facilitate liaisons between the persons we support and identified external supports.

To provide learning opportunities which enable staff to develop and maintain knowledge and skills.

To provide on-going internal and external evaluations and reviews of the support program, so as to ensure client satisfaction and quality of support.

To support staff through a team approach based on mutual respect and communication.

Eligibility
Eligibility is determined by the Department of Opportunities and Social Development, Disability Support Program.
A participant must have the ability to:
a) be alone at home and in the community for periods of time;
b) independently evacuate from their home in the event of an emergency;
c) access crisis support and/or use an emergency response system;
d) manage their medication, and other personal safety risks to themselves, others or their environment, safely and reliably, either independently or with available supports;
e) communicate their needs and preferences to others, and participate in decisions about their needs and activities; and
f) participate in the individual assessment and support planning processes.
